Coordinated Family Support (CFS)
We provide services to adult individuals living in their family homes. These services are adapted specifically to the language and culture of the home. The intent of these supports is to assist the individual in developing skills in the home, coordinating and delivering services, and accessing their community.

Community Day Services
Our community day service program empowers individuals with developmental disabilities to lead independent, fulfilling lives. We provide a supportive and inclusive environment where participants can enhance their quality of life, develop life skills, and foster personal growth. Through tailored activities, supported and competitive employment, paid internships, and community integration, we create an inclusive experience that supports each individual's unique needs and goals.

PROGRAM INCLUDES:

Developing and maintaining self-help and self-care skills.

Developing the ability to interact with others, making one’s needs known, and responding to instructions.

Developing self-advocacy and employment skills.

Developing community integration skills such as accessing community services.

Independent Living Services (ILS)
We provide training and support to help individuals with developmental disabilities build the skills necessary to live independently in their homes or community settings. These services focus on money management, meal preparation, household maintenance, personal health, and safety while fostering self-advocacy and decision-making. ILS aims to enhance the participant’s quality of life by promoting greater autonomy and the ability to manage daily activities independently.

COORDINATED CAREER PATHWAYS (CCP)
Our Coordinated Career Pathways (CCP) program is designed to help individuals with developmental disabilities achieve competitive, integrated employment and advance in their careers. We focus on creating personalized pathways based on each individual’s strengths, interests, and abilities, while addressing any barriers they may face in securing and maintaining employment.

CCP Services Include:

Career Exploration: Explore various career options through job shadowing, internships, and hands-on learning opportunities.

Vocational Training: Build job-specific skills to improve employability and workplace readiness.

Job Placement Support: Receive assistance in finding and securing competitive employment within the community.

Ongoing Support: Benefit from job coaching, workplace accommodations, and other support to ensure long-term workplace success.

Skill Development: Enhance essential soft skills, including communication, teamwork, and problem-solving, to thrive in the workplace.​

Our CCP program’s goal is to empower individuals with developmental disabilities to secure meaningful, sustainable employment while promoting independence and inclusion. Each service is person-centered and tailored to meet the unique needs of every participant.
